Please initial each section, sign/date at the end, and return this form to the teacher. By signing this agreement, you agree to respect and abide by the studio policies.
_____ General Information | Weekly private lessons are offered in 30, 45, or 60-minute sessions, depending on the student’s age/level.
_____ At the Studio | Students are expected to arrive for lessons in a timely manner, with all required materials and music. Parents and siblings may wait quietly in the library or entrance, wait in their vehicle, or run errands so long as they return to pick up their student in a timely fashion. To discuss student progress, parents are welcome to talk with the teacher during the first five minutes of the student’s lesson — but not afterwards, in order to respect the next student’s time slot.
_____ Practice and Instrument | Private students are expected to practice daily on a well-maintained acoustic piano or an electric keyboard with touch sensitive keys. Weighted keys on electric keyboards are preferable, but not essential for beginners.
_____ Materials | Students are recommended to use a canvas tote bag to carry books to and from lessons. The teacher will select appropriate materials for the student, and will give all pertinent information to the student/parent (usually via email) so that the materials may be purchased by the student/parent. This will include, but is not limited to, an assignment notebook which will be used keep track of assigned pieces and practice suggestions/goals for each week, lesson books, and theory books.
_____ Calendar and Schedule | Lessons will continue from September 10, 2012 through June 24, 2013, and can continue through the summer if desired. There are no lessons on statutory holidays, during the Winter break (December 24 – January 4), during Reading Break (February 18-23), or during Spring Break (April 1-5). There is a specific list of dates at the bottom, also available at katespianostudio.ca/calendar.
_____ Tuition and Payment | Lessons are $20/half hour, but students will be charged a regular monthly cost of $72/month ($70/month for Monday students), regardless of the number of lessons in each month. Payment will be made at the first lesson of each month, via cash, cheque, or online money transfer (send to email@example.com). Receipts are available upon request. The teacher reserves the right to raise fees at the beginning of each new academic year.
_____ Summer Months | The teacher is available to teach lessons in the summer and recommends this if the student is able and willing to practice. Lessons will be scheduled around the student’s and the teacher’s availability (e.g. vacations, summer camps, etc.), cancelations will be more flexible, and all students will be on the same day unless there are enough to fill two days. Notification of summer lesson plans should be made by June 21, 2013.
_____ Missed Lessons | There will be no reimbursement for missed lessons, but the teacher will do her best to reschedule the lesson, as long as she is given more than 24 hours notice. In the case of more than 24 hours notice for a missed lesson, please look at my lesson availability and select a time, and then contact me to confirm that the change is possible.
_____ Makeup Lessons | Due to the teacher’s limited availability, makeup lessons cannot be guaranteed. However, if an absence arises due to unforeseen situations such as an accident or family emergency and if the student indicates a desire for a makeup lesson, the teacher will make an effort to accommodate a makeup.
_____ Discontinuing Lessons | If a student and/or parent should decide to terminate lessons at any given time during the year, the studio requires a two week written notice in person or via email. Regardless of when appropriate notice is given, the student/parent will be financially responsible for the following two weeks/lessons, whether the student/parent chooses to attend or be absent.
_____ Parent Participation | Parents are encouraged to be supportive and active participants in practice at home. Parents are free to attend and quietly observe lessons if they wish.
- Studio Newsletters: Occasional studio newsletters will arrive in your student’s practice record book.
- Facebook Page: I have created a facebook page (facebook.com/katespianostudio) which students and parents are welcome to join. “Liking” this page allows students to interact with the teacher and other students while maintaining the privacy of each student’s facebook profile. For the most part, this is a page where I post music related content, including piano tutorials that I create.
- YouTube Channel: I created a YouTube channel in order to share my piano tutorials with the world, available at youtube.com/katespianostudio. It is a great resource for learning to play a few popular songs.
- Contacting the Teacher: The best way to contact the teacher is via email, which will be promptly returned with an email reply or phone call. Phone calls and text messages are also welcome from parents or students.
